Pre-Admission & Admission

If your scheduled hospital admission appointment has been arranged, you must complete a prior admission, also known as “pre-admission”. Two options are available to you.

Option 1 — Online pre-admission

You can complete your pre-admission directly online via the AP-HP portal:

For this online pre-admission, please prepare the following supporting documents:

  • A photo ID
  • Proof of entitlement to French Social Security
  • If applicable, a copy of both sides of your supplementary health insurance card

⚠️ This form is available until 5 days before your visit to the hospital (except for teleconsultations and regularisation after an Emergency Department visit). After this deadline, you will need to go directly to the administrative reception/admissions desk, unless you have received a message authorising you to go directly to the care unit.

Option 2 — On-site pre-admission

You may come to the hospital before the day of your admission to prepare your file: Social Security coverage, preparation of your medical file, etc. You may also complete your pre-admission after your appointment with the anaesthetist.

Go to the Admissions Department — Yellow sector, door 6.

Preparing your file in advance allows you, on the day of your appointment, to be directed straight to the department that will take care of you. This will save you time and help avoid forgetting required documents, which could jeopardise your hospital admission or consultation.

If you do not complete pre-admission

If you have not completed pre-admission, you will need to complete these formalities on the day of your hospital admission or consultation, by arriving at least 30 minutes before your appointment time and allowing for possible queues during busy periods.
