At the <a href=\"https:\/\/www.neurochirurgie-lariboisiere.com\/en\/healthcare\/skull-base-surgery\/what-is-skull-base-surgery\/\" target=\"_blank\" rel=\"noopener\">skull base<\/a>, it arises from the cartilage of the petroclival synchondrosis. Although histologically cancerous, it is most often a <span class=\"mc\">slow-growing<\/span> tumor \u2014 though sometimes aggressive. Its natural history and the factors influencing prognosis remain imperfectly understood.   \n  <\/div>\n\n  <!-- \u00c9PID\u00c9MIO -->\n  <section id=\"epidemio\" class=\"section-block\">\n    <h3>Who's affected?<\/h3>\n    <p>A very rare tumor, accounting for <strong>6% of skull base tumors<\/strong>. Patients are most often between 30 and 50 years old, both women and men. In the vast majority of cases, chondrosarcomas are isolated but can occur in the context of diseases such as <em>Ollier's disease<\/em> or <em>Paget's disease<\/em>.  <\/p>\n  <\/section>\n\n  <!-- SYMPT\u00d4MES -->\n  <section id=\"symptomes\" class=\"section-block\">\n    <h3>Symptoms<\/h3>\n    <p>The tumor develops slowly, with symptoms generally appearing late. At the skull base, the most frequent symptoms are <strong>headaches<\/strong> and <strong>double vision<\/strong>. Others may occur: visual or hearing loss, difficulty swallowing, hoarse voice, gait disturbances, motor weakness.  <\/p>\n  <\/section>\n\n  <!-- DIAGNOSTIC -->\n  <section id=\"diagnostic\" class=\"section-block\">\n    <h3>Diagnosis<\/h3>\n    <p>CT scans and especially <strong>MRI<\/strong> allow for a strong suspicion of the diagnosis thanks to the characteristic appearance and typical location of these tumors. As it is a rare tumor, diagnosis requires <strong>experienced radiologists<\/strong> \u2014 the distinction between chondrosarcoma and chordoma can be difficult on imaging. Clear cell and dedifferentiated forms are exceptional at the skull base. <strong>Mesenchymal<\/strong> forms are rarer and more aggressive, with a high risk of recurrence.  <\/p>\n\n    <div class=\"types-grid\">\n      <div class=\"type-card\">\n        <h4>Conventional Chondrosarcomas<\/h4>\n Subdivided into grades I, II, and III. Grades I and II are the most common. The <strong>risk of recurrence increases with grade<\/strong>.  \n      <\/div>\n      <div class=\"type-card\">\n        <h4>Rare Forms<\/h4>\n Clear cell, mesenchymal (more aggressive), and dedifferentiated chondrosarcomas \u2014 exceptional at the skull base.\n      <\/div>\n    <\/div>\n  <\/section>\n\n  <!-- TRAITEMENT -->\n  <section id=\"traitement\" class=\"section-block\">\n    <h3>Treatment<\/h3>\n\n    <h4>Multidisciplinary approach<\/h4>\n    <p>A <a href=\"https:\/\/www.neurochirurgie-lariboisiere.com\/rcp\/\" target=\"_blank\" rel=\"noopener\"><strong>multidisciplinary approach<\/strong><\/a> is crucial. A monthly national multidisciplinary team meeting (MDT) brings together all involved stakeholders. We collaborate with:  <\/p>\n\n    <ul style=\"margin-bottom: 20px;\">\n      <li>ENT Department and Neuroradiology Department of Lariboisi\u00e8re Hospital<\/li>\n      <li><a href=\"https:\/\/curie.fr\/liste\/centre-de-protontherapie\" target=\"_blank\" rel=\"noopener\">Orsay Proton Therapy Department<\/a> \u2014 Institut Curie (Dr. Hamid Mammar, Dr. Valentin Calugaru)<\/li>\n      <li>Institut Gustave Roussy (Dr. St\u00e9phanie Bolle)<\/li>\n    <\/ul>\n\n    <div style=\"text-align:center; margin-bottom: 32px;\">\n      <a href=\"http:\/\/neurochirurgie-lariboisi%C3%A8re.com\/rcp\" target=\"_blank\" rel=\"noopener\" style=\"display:inline-block; background:var(--bleu-accent); color:white; padding:12px 24px; border-radius:5px; font-weight:bold; text-decoration:none; font-size:0.9em; letter-spacing:0.03em;\">\n SKULL BASE MDT \u2014 CHONDROSARCOMAS \u2192\n      <\/a>\n    <\/div>\n\n    <ul class=\"traitements-list\">\n      <li>\n        <strong>Biopsy<\/strong>\n Rarely necessary. However, it can be useful when the distinction between chondrosarcoma and chordoma is difficult, in order to adapt the surgical strategy. \n      <\/li>\n      <li>\n        <strong>Surgery<\/strong>\n Must be performed in <strong>experienced centers<\/strong> for skull base neurosurgery. The objective is to remove as much of the tumor as possible while preserving critical nervous and vascular structures in the immediate vicinity. Excision also prepares the ground for complementary radiotherapy.  \n      <\/li>\n      <li>\n        <strong>Proton Therapy \/ Radiotherapy<\/strong>\n The 5-year recurrence risk is approximately <strong>30 to 40%<\/strong>. Depending on the degree of resection and tumor grade, high-dose radiotherapy is indicated. The reference technique is <a href=\"https:\/\/curie.fr\/liste\/centre-de-protontherapie\" target=\"_blank\" rel=\"noopener\">proton therapy (Orsay center)<\/a> \u2014 Dr. Mammar, Dr. Calugaru, Dr. Bolle. Hypofractionated stereotactic radiosurgery may be offered for small volume residuals or localized recurrences.   \n      <\/li>\n      <li>\n        <strong>Chemotherapy<\/strong>\n Reserved for aggressive and recurrent forms that cannot be reoperated.
