They result from aberrant inclusion of cutaneous tissue inside the cranial cavity during neural tube closure (3rd\u20135th embryonic week). This tissue then behaves like skin\u2014producing squamous debris\u2014but in the wrong location. They represent <span class=\"mc\">0.3 to 1.8% of all intracranial neoplasms<\/span> and are located in the subarachnoid spaces, around the brain, brainstem, and cerebellum.   \n  <\/div>\n\n  <!-- DIAGNOSTIC -->\n  <section id=\"diagnostic\" class=\"section-block\">\n    <h3>Diagnosis<\/h3>\n    <p>On CT scan, the epidermoid cyst appears as an iso- or hypodense mass, heterogeneous, with irregular contours, without contrast enhancement or perilesional edema.<\/p>\n    <p>The diagnosis is confirmed by <a href=\"https:\/\/patient.radiologie.fr\/fr\/examens-en-pratique\/irm\" target=\"_blank\" rel=\"noopener\"><strong>MRI<\/strong><\/a>, and particularly by <strong>hyperintensity on diffusion-weighted sequences<\/strong>\u2014sequences that are essential for confirming the diagnosis with certainty.<\/p>\n  <\/section>\n\n  <!-- TRAITEMENT -->\n  <section id=\"traitement\" class=\"section-block\">\n    <h3>Treatment<\/h3>\n    <p>With very slow progression, they are most often <strong>monitored with regular MRI scans<\/strong>. Surgery is recommended in case of symptoms. If symptoms are moderate and the operative risk is significant, surveillance may be preferred initially.  <\/p>\n\n    <ul class=\"traitements-list\">\n      <li>\n        <strong>Complete resection\u2014primary objective<\/strong>\n The treatment of choice is total resection of the tumor, including the capsule. Since the capsule produces the cystic content, leaving it in place carries a <strong>high risk of recurrence<\/strong>. Complete resection is particularly important during the first intervention, as postoperative scarring makes any subsequent surgery even more difficult.  \n      <\/li>\n      <li>\n        <strong>Partial resection<\/strong>\n Sometimes unavoidable when the cyst is in close proximity to critical neural or vascular structures. Part of the capsule is then left in place, with a significant risk of long-term recurrence. \n      <\/li>\n      <li>\n        <strong>Surgical approach<\/strong>\n It depends on the location of the cyst. Some large cysts require <a href=\"https:\/\/www.neurochirurgie-lariboisiere.com\/en\/healthcare\/skull-base-surgery\/what-is-skull-base-surgery\/\" target=\"_blank\" rel=\"noopener\">complex skull base approaches<\/a> to allow complete removal while preserving neural and vascular structures. \n      <\/li>\n    <\/ul>\n  <\/section>\n\n  <!-- \u00c9VOLUTION -->\n  <section id=\"evolution\" class=\"section-block\">\n    <h3>Course and prognosis<\/h3>\n    <p>Progression is very slow. After surgery, if part of the capsule is left in place, the risk of recurrence is significant\u2014often after many years. Reoperation is sometimes necessary.  <\/p>\n    <p>The overall prognosis is <span class=\"mc\">favorable<\/span>, but these cysts often require complex intervention that is not without risks, requiring <strong>specific surgical expertise<\/strong>. It is more variable, more heterogeneous, due to the diversity of materials that collect in its cavity. The epithelial lining is identical, but its wall also contains dermal elements: hair follicles, sebaceous and sweat glands, and sometimes even fat lobules. The cystic content is thus a more or less heterogeneous amalgam of keratin, cholesterol, sebaceous and sweat secretions, giving a more yellowish appearance, often with hairs.<\/p><\/div><div
