Some can lead to serious neurological consequences in the event of bleeding, while others present a low hemorrhagic risk and remain benign. \n<\/div>\n\n<section class=\"section-block\">\n<h3>How do they manifest themselves?<\/h3>\n\n<p>Vascular malformations may present with hemorrhage:<\/p>\n\n<ul class=\"traitements-list\">\n  <li><strong>Intracerebral or subarachnoid<\/strong> \u2014 in the brain<\/li>\n  <li><strong>Perimedullary or intramedullary<\/strong> (hematomyelia) \u2014 in the spinal cord<\/li>\n<\/ul>\n<\/section>\n\n<section class=\"section-block\">\n<h3>Therapeutic options<\/h3>\n\n<p>When treatment is necessary, several options may be offered. The choice depends on the type, size, configuration of the malformation, and the patient's general condition. <\/p>\n\n<div class=\"types-grid\">\n<div class=\"type-card\">\n<h4>Surgery<\/h4>\nIndicated for accessible malformations, particularly in cases of bleeding or high hemorrhagic risk.\n<\/div>\n<div class=\"type-card\">\n<h4>Endovascular Treatment<\/h4>\nPerformed from inside the vessels (embolization), alone or in combination with surgery or radiotherapy.\n<\/div>\n<div class=\"type-card\">\n<h4>Radiotherapy<\/h4>\nOption for deep or surgically inaccessible malformations, particularly through high-precision radiosurgery.\n<\/div>\n<div class=\"type-card\">\n<h4>Combined Approach<\/h4>\nSome complex malformations require a combination of several therapeutic modalities, discussed collegially.\n<\/div>\n<\/div>\n\n<p>Each case is discussed in a <strong>multidisciplinary consultation<\/strong> to provide the most appropriate care. Some present suddenly with hemorrhage, others progressively, and some remain completely <span class=\"mc\">asymptomatic<\/span> and benign. \n<\/div>\n\n<section class=\"section-block\">\n<h3>1. Arteriovenous Malformations and Cerebral Dural Fistulas<\/h3>\n\n<p>These malformations may present suddenly with hemorrhage, the location of which varies:<\/p>\n\n<ul class=\"traitements-list\">\n  <li><strong>Subarachnoid hemorrhage (or meningeal)<\/strong> \u2014 bleeding around the brain, in the subarachnoid spaces<\/li>\n  <li><strong>Cerebral or cerebellar hemorrhage<\/strong> \u2014 bleeding inside the brain or cerebellum<\/li>\n  <li><strong>Cerebromeningeal hemorrhage<\/strong> \u2014 combining bleeding in the subarachnoid spaces and in the cerebral tissue (hematoma)<\/li>\n<\/ul>\n\n<p>These malformations may also be <strong>discovered incidentally<\/strong>, during a CT scan or MRI performed for another reason, or manifest through <strong>epileptic seizures<\/strong> caused by cortical irritation related to changes in blood flow.<\/p>\n<\/section>\n\n<section class=\"section-block\">\n<h3>2. Intracerebral or Spinal Cavernomas<\/h3>\n\n<p>Cavernomas may manifest with <span class=\"mc\">sudden bleeding<\/span> that can cause neurological deficit depending on their location, or with an <strong>epileptic seizure<\/strong>.<\/p>\n<\/section>\n\n<section class=\"section-block\">\n<h3>3. Isolated Venous Angiomas<\/h3>\n\n<p>Isolated venous angiomas are congenital and <strong>completely asymptomatic<\/strong> \u2014 they are therefore benign and require no treatment. They may be associated with a cavernoma, which can explain the presence of symptoms in case of bleeding from the latter. <\/p>\n<\/section>\n\n<section class=\"section-block\">\n<h3>4. Cerebral Telangiectasias<\/h3>\n\n<p>A cerebral telangiectasia is a small malformation of the brain capillaries corresponding to an abnormal dilation of these vessels. It forms a fragile but <strong>generally benign<\/strong> area, requiring no treatment. <\/p>\n<\/section>\n\n<section class=\"section-block\">\n<h3>5. Spinal Dural Fistulas<\/h3>\n\n<p>Spinal dural fistulas do not bleed but cause the <strong>progressive<\/strong> appearance of:<\/p>\n\n<ul class=\"traitements-list\">\n  <li><strong>Sciatic-type pain<\/strong><\/li>\n  <li><strong>Weakness when walking<\/strong><\/li>\n  <li><strong>Sphincter disorders<\/strong> progressively worsening<\/li>\n<\/ul>\n\n<p>There is often a <span class=\"mc\">diagnostic delay<\/span> as other pathologies (particularly lumbar hernia) may be suspected. A <strong>complete spinal MRI (panmedullary)<\/strong> is necessary to guide the diagnosis, then confirmed by <strong>spinal arteriography<\/strong>. <\/p>\n<\/section>\n\n<section class=\"section-block\">\n<h3>6.
