Much rarer than cerebral malformations, their incidence is estimated at <span class=\"mc\">2 to 11% of all cerebro-spinal arteriovenous malformations<\/span>. <\/div><section class=\"section-block\"><h3>Classification of spinal vascular malformations<\/h3><p>Spinal vascular malformations are classified into four types:<a href=\"https:\/\/www.neurochirurgie-lariboisiere.com\/wp-content\/uploads\/2026\/03\/arterio-scanner.jpg\"><img decoding=\"async\" class=\"alignleft\" src=\"https:\/\/www.neurochirurgie-lariboisiere.com\/wp-content\/uploads\/2026\/03\/arterio-scanner.jpg\" alt=\"Arteriography of a spinal vascular malformation\" width=\"161\" height=\"195\"><\/a><\/p><ul class=\"traitements-list\"><li><strong>Type I \u2014 Dural arteriovenous fistula<\/strong><br>The most common form, representing approximately 60 to 80% of cases.<\/li><li><strong>Type II \u2014 Intramedullary arteriovenous malformation<\/strong><br>The nidus is located in the parenchyma of the spinal cord, typically supplied by the anterior spinal artery, in front of the cord.<\/li><li><strong>Type III \u2014 Intramedullary arteriovenous malformation with extramedullary extension<\/strong><br>The feeding vessels originate from the vertebral plexuses, with extension beyond the spinal cord.<\/li><li><strong>Type IV \u2014 Intradural perimedullary fistula<\/strong><br>Supplied by the anterior spinal artery, it drains directly into a perimedullary vein (approximately 20% of cases).<\/li><\/ul><\/section><section class=\"section-block\"><h3>How does a spinal vascular malformation present?<\/h3><div class=\"types-grid\"><div class=\"type-card\"><h4>Dural and perimedullary fistulas<\/h4><p>Often revealed by progressive symptoms: gait disturbances, sphincter dysfunction (myelopathy), or atypical sciatic-type pain (radicular syndrome).<\/p><\/div><div class=\"type-card\"><h4>Other malformations<\/h4><p>May bleed with a risk of sudden deficit, or even <span class=\"mc\">paraplegia or tetraplegia<\/span>. They may also present as subarachnoid hemorrhage. <\/p><\/div><\/div><\/section><section class=\"section-block\"><h3>How are they treated?<\/h3><p><strong>Arteriography<\/strong> is mandatory for the diagnosis of all these spinal vascular malformations.
