These are very slow-growing tumors (~4 mm\/year), which gives the brain the opportunity to adapt and reorganize its circuits. <strong>Prof. Mandonnet<\/strong> has specialized in their management for more than 20 years, with a <span class=\"mc\">personalized, patient-centered<\/span> approach. The entire care pathway is coordinated by a dedicated medical assistant: <strong>Ms. Chadia Draou<\/strong> \u2014 <a href=\"tel:0149958146\">01 49 95 81 46<\/a> \u00b7 <a href=\"mailto:chadia.draou@aphp.fr\">chadia.draou@aphp.fr<\/a>   \n  <\/div>\n\n  <!-- PARCOURS DE SOINS -->\n  <section id=\"parcours\" class=\"section-block\">\n    <h3>Care pathway<\/h3>\n\n    <ol class=\"examens\">\n      <li>\n        <h4>At the time of diagnosis<\/h4>\n        <p>The first consultation with Prof. Mandonnet makes it possible to get acquainted and to explain the disease and its treatment options: surgery, chemotherapy (Temodal or PCV), radiotherapy, targeted therapy (Vorasidenib, Ivosidenib). Surgery is most often proposed as first-line treatment because of its <strong>proven benefit<\/strong>. The principle of a <em>functionally maximal resection<\/em> is explained in detail, as well as how an awake procedure is carried out.  <\/p>\n        <p>Transient disorders may appear in the immediate postoperative period, but recover remarkably thanks to brain plasticity and cognitive rehabilitation (PRM department of Prof. Beaudreuil). The vast majority of patients return to work after 3 to 6 months of rehabilitation (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/38372820\/\" target=\"_blank\" rel=\"noopener\">article<\/a>). However, significant fatigue may persist for a few months, which may require a return on a part-time therapeutic schedule (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35462205\/\" target=\"_blank\" rel=\"noopener\">article<\/a>).  <\/p>\n      <\/li>\n      <li>\n        <h4>Preparation for awake surgery<\/h4>\n        <p>A comprehensive assessment is carried out before the procedure:<\/p>\n        <ul>\n          <li><strong>Language and cognitive assessment<\/strong> with a speech therapist from the team (Isabelle Poisson, Marion Barberis, C\u00e9cile Prevost, Sophie Bouteloup). Essential for identifying subtle vulnerabilities indicating that the brain has reached the limits of its compensatory capacity. <\/li>\n          <li><strong>Specialized functional and anatomical imaging<\/strong> \u2014 to confirm language-function lateralization and select appropriate intraoperative monitoring tests.<\/li>\n          <li><strong>Electroencephalogram (EEG)<\/strong> \u2014 to optimize adjustment of anti-seizure medications.<\/li>\n          <li><strong>Preoperative consultations<\/strong> with the anesthesiologist and nurse anesthetist, to prepare for the hypno-sedation technique used at Lariboisi\u00e8re.<\/li>\n        <\/ul>\n      <\/li>\n      <li>\n        <h4>During surgery<\/h4>\n        <p><strong>Awake surgery<\/strong> is one of the most reliable methods for maximizing resection while preserving function. The procedure lasts approximately 5 to 6 hours, including 2 hours awake during which the patient performs various tasks under the guidance of the speech therapist. The entire operating-room team supports and encourages the patient throughout the procedure.  <\/p>\n      <\/li>\n      <li>\n        <h4>After surgery<\/h4>\n        <p>The pathology results (Prof. Biassette\u2019s department) are available within 5 to 10 days. Depending on the tumor type and grade, additional treatment may be proposed: radiotherapy, chemotherapy, or targeted therapy. In high-grade tumors (glioblastomas), chemoradiotherapy is almost systematic. Enrollment in promising <strong>clinical trials<\/strong> is offered whenever possible.   <\/p>\n      <\/li>\n      <li>\n        <h4>Further treatment<\/h4>\n        <p>In the vast majority of cases, a period of <strong>clinical and radiological surveillance<\/strong> is instituted. Adjuvant treatment is proposed only in the event of progression or signs of aggressiveness, after discussion at the multidisciplinary tumor board (neuro-oncologist, radiation oncologist, radiologist, pathologist, molecular biologist, neurosurgeon). Options include:  <\/p>\n        <ul>\n          <li>Chemotherapy with the PCV regimen (procarbazine \u2013 CCNU \u2013 vincristine)<\/li>\n          <li>Radiotherapy<\/li>\n          <li>Targeted anti-IDH mutation therapy (Vorasidenib, Ivosidenib)<\/li>\n          <li>Repeat surgery<\/li>\n        <\/ul>\n      <\/li>\n    <\/ol>\n  <\/section>\n\n  <!-- RECHERCHE -->\n  <section id=\"recherche\" class=\"section-block\">\n    <h3>Search<\/h3>\n\n    <ul class=\"traitements-list\">\n      <li>\n        <strong>Digital tools for cognitive assessment \u2014 LABCOM Cog-Toolkit<\/strong>\n Joint laboratory between Frontlab at the Brain Institute and the company Humans Matter. The Cog-Surg platform, dedicated to awake surgery, is currently under development.\n      <\/li>\n      <li>\n        <strong>Prediction of post-surgical cognitive functions<\/strong>\n Prof. Mandonnet\u2019s work on understanding brain plasticity: cognitive flexibility (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35245898\/\" target=\"_blank\" rel=\"noopener\">publication<\/a>) and creativity (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/38593576\/\" target=\"_blank\" rel=\"noopener\">publication<\/a>).
