Although benign, these tumors can behave aggressively locally. \n<\/div>\n\n<section class=\"section-block\">\n<h3>Definition<\/h3>\n\n<p>As it grows, a craniopharyngioma can affect pituitary function, vision, and sometimes hypothalamic function. When large, these tumors can block the flow of cerebrospinal fluid and lead to hydrocephalus (enlargement of the brain\u2019s ventricles). <\/p>\n\n<p>Craniopharyngiomas may consist of three components:<\/p>\n\n<ul class=\"traitements-list\">\n  <li><strong>Composante solide<\/strong> - partie charnue de la tumeur<\/li>\n  <li>Cystic component - the fluid-filled part of the tumor<\/li>\n  <li><strong>Calcified component<\/strong><\/li>\n<\/ul>\n\n<p>Unlike other tumors, craniopharyngiomas do not always follow a linear course: they may grow in size, remain stable, or even shrink. Watchful waiting may therefore be recommended if the tumor is asymptomatic or causes only mild symptoms. <\/p>\n\n<p>From a surgical standpoint, these tumors require neurosurgeons who specialize in the sella region and are proficient in all access techniques:<\/p>\n\n<ul class=\"traitements-list\">\n  <li><strong>Endonasal endoscopy<\/strong><\/li>\n  <li><strong>Transcranial microsurgery<\/strong><\/li>\n<\/ul>\n\n<p>Care is necessarily multidisciplinary, involving neurosurgeons, endocrinologists, ophthalmologists, neuroradiologists, pathologists, and radiation oncologists.<\/p>\n<\/section>\n\n<section class=\"section-block\">\n<h3>Frequency and age of onset<\/h3>\n\n<p>Craniopharyngioma is a rare tumor, with an estimated incidence of 1 to 2 cases per million people per year. It accounts for 2% of all brain tumors and up to 5 to 10% of brain tumors in children. Both sexes are affected equally.  <\/p>\n\n<p>There are two distinct age groups in which the condition most commonly occurs:<\/p>\n\n<div class=\"types-grid\">\n<div class=\"type-card\">\n<h4>Pediatric formulation<\/h4>\nOnset between the ages of 5 and 14. Treated at the pediatric neurosurgery department at Necker Hospital, a referral center for pediatric craniopharyngiomas. \n<\/div>\n<div class=\"type-card\">\n<h4>Adult form<\/h4>\nOnset between the ages of 50 and 70. Treated at the Lariboisi\u00e8re Neurosurgery Department, a center of expertise for craniopharyngiomas in adults. \n<\/div>\n<\/div>\n\n<p>This dual approach underscores the importance of close collaboration between adult and pediatric services, particularly when it comes to coordinating the transition from childhood to adulthood.<\/p>\n<\/section>\n\n<section class=\"section-block\">\n<h3>Two histological types<\/h3>\n\n<p>Pathological analysis (microscopic examination of tissue samples taken in the operating room) distinguishes between two types of craniopharyngiomas:<\/p>\n\n<div class=\"types-grid\">\n<div class=\"type-card\">\n<h4>Adamantine craniopharyngioma<\/h4>\nThe most common. A tumor that is often calcified and occurs in both children and adults. \n<\/div>\n<div class=\"type-card\">\n<h4>Papillary craniopharyngioma<\/h4>\nRarer, exceptionally calcified, occurring only in adults. An endocrinology consultation completes the management (hormonal replacement therapy if necessary). <\/p>\n\n<h4>Radiological workup<\/h4>\n<p><strong>Pituitary MRI<\/strong> is the reference examination \u2014 it analyzes the extensions of the craniopharyngioma. A <strong>CT scan<\/strong> (angio-CT, bone windows) may complement the evaluation, particularly to identify <span class=\"mc\">calcifications<\/span>. <\/p>\n\n<h4>Ophthalmological workup<\/h4>\n<p>Necessary due to the proximity of the visual pathways. It is based on three pillars: surgery, hormonal treatment, and radiotherapy. \n<\/div>\n\n<section class=\"section-block\">\n<h3>1. Surgical treatment<\/h3>\n\n<p>If the craniopharyngioma is minimally or not symptomatic, <strong>initial surveillance<\/strong> may be proposed. However, if it becomes progressive or symptomatic (visual disturbances, hypothalamic disorders), surgical intervention is necessary in a <span class=\"mc\">specialized center<\/span>. <\/p>\n\n<p>The objective is to remove the maximum amount of tumor while <strong>preserving hypothalamic function<\/strong>. When the tumor infiltrates the hypothalamus, a residual portion is generally left in place \u2014 this may be treated with radiotherapy if it is progressive or threatens the visual pathways. It is frequently impossible to preserve pituitary function, which requires postoperative <strong>hormonal replacement therapy<\/strong>.  <\/p>\n\n<div class=\"types-grid\">\n<div class=\"type-card\">\n<h4>Endoscopic endonasal approach<\/h4>\nApproach through the nose, preferred in the <strong>majority of cases<\/strong>. Minimally invasive technique allowing direct access to the sellar and suprasellar region. \n<\/div>\n<div class=\"type-card\">\n<h4>Transcranial microsurgery<\/h4>\nSuperior approach, indicated more rarely depending on tumor configuration. Requires complete mastery of both approaches. \n<\/div>\n<\/div>\n\n<p>This is delicate surgery, but associated with a <strong>low complication rate<\/strong> in experienced teams.<\/p>\n<\/section>\n\n<section class=\"section-block\">\n<h3>2. Hormonal treatment<\/h3>\n\n<p>Management of a craniopharyngioma requires the presence of an <strong>endocrinologist<\/strong> at every stage \u2014 diagnosis, follow-up, and postoperative care. Hormonal deficits are indeed very common, whether present at diagnosis or occurring after surgery. <\/p>\n\n<p>Endocrinological management allows for <strong>detection and replacement<\/strong> of these deficits with appropriate medication (medication replacing the hormone that the pituitary gland can no longer secrete).<\/p>\n<\/section>\n\n<section class=\"section-block\">\n<h3>3.
