As the true \"conductor\" of the hormonal glands, the pituitary regulates numerous bodily functions. <\/div>\n<section class=\"section-block\">\n<h3>Definition and Classification<\/h3>\nThe management of adenomas is specialized and multidisciplinary. When surgery is indicated, the expertise of the neurosurgeon is essential. In an expert center, the outcome is favorable in the vast majority of cases.  \n\nHormonal assessment (blood test) allows classification of the adenoma into two main categories:\n<div class=\"types-grid\">\n<div class=\"type-card\">\n<h4>Secreting Adenoma<\/h4>\nExcessive secretion of a pituitary hormone:\n<ul style=\"margin: 8px 0 0; padding-left: 18px; font-size: 0.93em;\">\n \t<li>Prolactin \u2192 <strong>Prolactinoma<\/strong><\/li>\n \t<li>ACTH\/Cortisol \u2192 <strong>Cushing's Disease<\/strong><\/li>\n \t<li>GH\/IGF-1 \u2192 <strong>Acromegaly<\/strong><\/li>\n<\/ul>\n<\/div>\n<div class=\"type-card\">\n<h4>Non-Functioning Adenoma<\/h4>\nAbsence of hormonal hypersecretion. The therapeutic strategy differs according to the volume and relationship of the lesion with neighboring structures, particularly the optic pathways. \n\n<\/div>\n<\/div>\nAccording to their size, they are classified as:\n<ul class=\"traitements-list\">\n \t<li><strong>Microadenomas<\/strong> (diameter &lt; 1 cm)\u2014often present with hormonal secretion issues (Cushing's disease, acromegaly, prolactinoma)<\/li>\n \t<li><strong>Macroadenomas<\/strong> (diameter \u2265 1 cm)\u2014primarily pose a mass effect with risk of <span class=\"mc\">compression of the visual pathways<\/span><\/li>\n<\/ul>\n<\/section><section class=\"section-block\">\n<h3>Frequency<\/h3>\nPituitary adenomas represent approximately <strong>15% of intracranial tumors<\/strong> in adults. Symptoms Related to Excessive Hormonal Secretion<\/h3>\n\n<p>These symptoms occur only if the adenoma is <strong>secreting<\/strong>.<\/p>\n\n<h4>Cushing's Disease\u2014Corticotroph Adenoma<\/h4>\n<p>Hypersecretion of ACTH stimulating adrenal cortisol production:<\/p>\n<ul class=\"traitements-list\">\n  <li>Weight gain predominantly in the face and trunk, obesity<\/li>\n  <li>Round, red face (facial erythrosis)<\/li>\n  <li>Hump at the back of the neck (<em>\"Buffalo Neck\"<\/em>)<\/li>\n  <li>Excessive hair growth (hirsutism), stretch marks, skin fragility<\/li>\n  <li>Muscle weakness in the lower limbs\u2014difficulty standing up, climbing stairs<\/li>\n  <li>High blood pressure, diabetes<\/li>\n  <li>Irritability, mood disorders, or even psychiatric disturbances<\/li>\n<\/ul>\n\n<h4>Acromegaly\u2014Somatotroph Adenoma<\/h4>\n<p>Hypersecretion of growth hormone:<\/p>\n<ul class=\"traitements-list\">\n  <li>Progressive thickening of the face and extremities (feet\/hands)<\/li>\n  <li>Thickening of soft tissues<\/li>\n  <li>Excessive growth of the mandible (prognathism), spacing of teeth, malocclusion<\/li>\n  <li>Excessive sweating, snoring, sleep apnea syndrome<\/li>\n  <li>Joint pain, carpal tunnel syndrome<\/li>\n  <li>High blood pressure, diabetes<\/li>\n<\/ul>\n\n<h4>Prolactinoma\u2014Lactotroph Adenoma<\/h4>\n<p>Hypersecretion of prolactin:<\/p>\n<ul class=\"traitements-list\">\n  <li>Absence of menstruation (secondary amenorrhea)<\/li>\n  <li>Milky discharge from the breasts (galactorrhea)<\/li>\n  <li>Sexual dysfunction (decreased libido)<\/li>\n<\/ul>\n\n<h4>Thyrotroph Adenoma <em>(very rare condition)<\/em><\/h4>\n<p>Central hyperthyroidism secondary to TSH hypersecretion by the adenoma.<\/p>\n<\/section>\n\n<section class=\"section-block\">\n<h3>2. Symptoms Related to Compression of Neighboring Structures<\/h3>\n\n<p>These symptoms can occur in all types of adenomas, provided the tumor is sufficiently large.<\/p>\n\n<ul class=\"traitements-list\">\n  <li><strong>Visual disturbances<\/strong> related to compression of the <span class=\"mc\">optic chiasm<\/span>\u2014reduced visual field, decreased visual acuity<\/li>\n  <li><strong>Headaches<\/strong><\/li>\n  <li><strong>Diplopia<\/strong> (double vision) and <strong>ptosis<\/strong> (drooping eyelid)\u2014related to compression of oculomotor nerves in the cavernous sinus; rare signs, most often suggesting necrosis or bleeding within the tumor<\/li>\n<\/ul>\n<\/section>\n\n<section class=\"section-block\">\n<h3>3. Symptoms Related to Compression of the Pituitary Gland<\/h3>\n\n<p>Compression of the healthy pituitary can lead to insufficient hormone secretion (hypopituitarism), generally manifesting as: fatigue, pallor, thin skin, sexual dysfunction.<\/p>\n\n<ul class=\"traitements-list\">\n  <li><strong>Corticotroph insufficiency<\/strong> (cortisol deficiency)\u2014the most dangerous, with life-threatening risk in case of decompensation: asthenia, low blood pressure, digestive disorders<\/li>\n  <li><strong>Thyrotroph insufficiency<\/strong> (thyroid hormone deficiency)\u2014pallor, weight gain, constipation, depression<\/li>\n  <li><strong>Gonadotroph insufficiency<\/strong> (sex hormone deficiency)\u2014hypogonadism: absence of puberty, amenorrhea, decreased libido, impotence, infertility<\/li>\n  <li><strong>Somatotroph insufficiency<\/strong> (growth hormone deficiency)\u2014in children: growth retardation<\/li>\n<\/ul>\n\n<p>In the presence of these symptoms, clinical analysis points toward pituitary pathology. If suspected, a <strong>hormonal assessment<\/strong> is performed as a priority. It provides detailed analysis of adenoma extensions: <\/p><ul class=\"traitements-list\"><li><strong>Within the sella turcica<\/strong>\u2014relationship with the pituitary gland<\/li><li><strong>Suprasellar extension<\/strong>\u2014relationship with the chiasm and optic nerves<\/li><li><strong>Infrasellar extension<\/strong>\u2014toward the sphenoid sinus<\/li><li><strong>Lateral extension<\/strong>\u2014toward the cavernous sinuses<\/li><\/ul><p>Depending on the case, a <strong>CT scan<\/strong> (angio-CT, bone windows) may supplement the evaluation and be used for <span class=\"mc\">neuronavigation<\/span> to guide surgery.<\/p><\/section><section class=\"section-block\"><h3>3. Ophthalmological Assessment<\/h3><p>This assessment is often necessary due to the proximity of the <strong>visual pathways<\/strong>.
